Documents are required to submit the form, so you need to keep scanned copies of your documents handy and attach them with the completed form.

The documents that you need when applying for an Indian Passport vary depending on the mode, location, marital status or the applicants’ profile. In the normal mode for an adult, you need to have a document stating your date of birth (age proof) and proof of address (residential proof). For age proof you need a birth certificate issued by a Municipal authority, a school leaving certificate or an affidavit sworn before a magistrate. 

For the residential proof, you will need a scanned copy of your water bill, telephone or electricity bill. Other documents that can fulfil this requirement are a 1 year statement with transactions and passbook, an income tax assessment order , certificate from employer (on an official letterhead) or the newly issued Aadhar Card. For marital status documentation, you need a scanned copy of your marriage certificate so that the name of your spouse can appear on your passport. In the case of minors, you need passport copies of the parents/guardians or affidavits from both parents to be considered.
